Free Health Check-up Coupon Details
  • One free health check-up coupon per policy
  • Any one member within the plan can avail of the free health check-up
  • The free health check-up coupon is valid for 365 days from the date of policy issuance
  • A system generated SMS will be sent to all customers
  • As informed in the SMS, The Customer will have to call up ICICI Lombard’s i Health Care appointment desk for Health check-up on 0120 4663015 between 10 AM to 5PM (Monday to Friday) and provide:
    • Policy number
    • Policy issue date
    • Preferred date of health check-up
  • After getting the center preference from the customer, and confirming the details, the appointment will be scheduled to the nearest network center.
  • Appointments can only be fixed (minimum) 2 days in advance.
  • The auto-generated authorization code will be provided to the customer through an SMS along with contact details of the test center.
  • The customer will have to fill up all the details on the coupon and carry it along with the Health card to the center. Without both, the center will not carry out the test.
  • The customer will have to hand over the coupon to the center on completion of the test.
  • The customer will have to go to the center latest by 10 am
  • The customer will have to be fasting at least for 10 hours before the test. Plain water may be consumed during this period. No tea, coffee to be consumed in the morning.
  • The customer will have to collect the reports from the test center itself.

    6. Fast Blood Sugar
    7. Urine Routine
