    According to the Motor Vehicles Act of 1988, it is essential to have a driving licence for two-wheeler and four-wheeler vehicles in India. One of the most vital documents in this process is RTO Form 4, which is important while applying for a driving licence.

    It is illegal to drive a motor vehicle in India if you do not have a driving licence. Therefore, to obtain it, you must follow certain procedures. You must apply for a learner’s licence first, and then after a month of its issuance, you can apply for a permanent driving licence.

    In this blog, we look at step-by-step methods on how to download the form, fill it out and submit it efficiently.

    What is RTO Form 4?

    Form 4 is a legal document that must be filled and submitted at the Regional Transport Office to apply for a driving licence in India. This application validates that the person can legally drive a motor vehicle within the boundaries of the country. Without RTO Form 4, it is impossible for you to obtain a driving licence. 

    Form 4 verifies the identity of the person and their appearance during the driving test. Remember, applicants must be 18 years or above to apply for a driving licence. 

    How to Fill RTO Form 4?

    Filling out RTO Form 4 accurately is essential for a hassle-free experience while applying for a driving licence. It has three categories:

    • The first part consists of the licensing authority’s address, type of vehicle and a box to attach your passport-size image. 
    • In the second part, you’ll need to fill in your personal details. 
    • The third part consists of a certificate of driving competency. 

    Here’s a step-by-step guide: 

    • Write down the address of the RTO, attach your passport-size photograph, and select the vehicle type for which you’re applying for a licence. 
    • Point 1, 2 and 3 requires your name, your father/husband’s name and permanent address. 
    • In case you’re residing at a temporary address, mention that in point 4, include the period of stay at the current location in point 5.
    • Fill your date and place of birth in point 6 and 7. 
    • People born outside India should mention their year of migration in point 8. 
    • Point 9 requires your educational qualifications. 
    • In point 10, mention two visible identification marks (birthmarks, moles on face, arms or legs). 
    • Mention your blood group in point 12. 
    • Point 13 requires you to mention if you already have a valid driving licence. If yes, you need to specify the vehicle category.
    • Enter information regarding the dates of all convictions that were ordered on any permits in your name in point 14.
    • In point 15, mention if you have been previously disqualified from obtaining a driving licence. If yes, state the reason in the second line.
    • Mention your physical fitness evaluation from a driving test in point 16. Mention the date, name of the testing authority, and the result.
    • Point 17 needs you to mention if you have submitted three passport-size photographs. 
    • In point 18, include the date and name of the issuing authority about your learner’s licence.
    • Mention the name and address of the driving school in point 19, if you have a driving certificate.
    • In point 20, you need to mention if you had submitted a parent/guardian consent form while applying for a learner's licence. Similarly, in point 21, mention if you provided a medical fitness certificate when the learner’s licence was issued. 
    • In point 22 and 23, mention whether you were excused from the medical and preliminary exam according to Rule 6 and Rule 11(2) of the Central Motor Vehicles Rules, 1989. 
    • Mention the fees paid while applying for a driving license in point 24. 

    The RTO official will sign the form, mentioning the date and registration number of the vehicle used for the exam. You must also sign here.

    Once the application is accepted, you will be informed via email or called at the RTO for further verification. 

    How to Download Form 4?

    Downloading the RTO Form 4 is quite simple with these steps:

    1. Visit the site- https://parivahan.gov.in/parivahan//en/content/download-forms
    2. Under the Driving Licence section, choose ‘Application for Licence to Drive’, and click on Form 4.
    3. Once redirected to Form 4, click on the download option. 

    Documents Required for Form 4

    You must attach the following supporting documents while filling out the RTO Form 4 to verify your application:

    Identity Proof

    Aadhaar card, Voter’s ID card, Passport, PAN card, Ration card

    Address Proof

    Aadhaar card, Electricity bill, Passport, Ration card, Telephone bill, Voter’s ID card

    Age Proof

    Birth Certificate, Affidavit from Executive or Judiciary Magistrate

    Qualification Proof

    Copy of Board/University certificates.
