Diagnosis Of PROM In Pregnancy

Premature rupture of membranes (PROM), a condition where the water (amniotic sac) around the baby breaks before labour starts. The most common sign of PROM is a slow leak of fluid from the vagina. The fluid is usually clear or slightly yellow. Some women may mistake it for urine or vaginal discharge.

If a pregnant woman notices these signs, she should immediately contact her doctor or midwife.

Diagnosis and tests of PROM?

Doctors use differential diagnosis of PROM. Some of the common ways to diagnose it are:

  • Physical examination: The medical examiner will inquire about symptoms and check if fluid leaks from the cervix (the womb's opening).
  • Sterile speculum examination: A speculum is a medical tool used to look inside the vagina. The doctor will check for signs of amniotic fluid pooling in the vagina.
  • Nitrazine test: A small sample of fluid is tested using a special paper that changes colour if amniotic fluid is present. However, this test can sometimes give false results if there is blood, semen, or an infection.
  • Ferning test: A sample of the fluid is examined under a microscope. If it forms fern-like patterns, it confirms that it is amniotic fluid.
  • Ultrasound scan: An ultrasound can check the amount of amniotic fluid around the baby. If there is less fluid than expected, it may suggest PROM.
  • AmniSure or Actim PROM test: Modern tests detect proteins found in amniotic fluid. These are highly accurate tests and provide quick results.
